Formation Proficiency Report (FPR)
This form should be used by all current FAST pilots to log currency flights. If you are transferring to the RPA, and have yet to fly with an RPA lead or check pilot, please use the Transferring FPR.

FAST Recommendation and Evaluation Form
This form should be presented to the RPA lead or check pilot when ready for a recommendation flight. After the evaluation, it should be mailed to our Chairman of FAST Records.
